Immunizations:

Required Vaccine
All students are required to meet the following Measles vaccine (MMR) immunization requirement before class registration:
  • Provide proof of two measles vaccinations (one of which must be dated after 1979), or a lab test showing proof of immunity to the measles (rubeola).
  • Submit the MMR Immunization Form (PDF). You will see a MMR Immunization hold under "My Tasks" on My ASU until your measles information is received and verified. Read additional information about MMR.
The MMR vaccine is available for a fee at any of ASU's health services locations.
Recommended Vaccine
ASU Health Services also strongly recommends immunization against meningococcal meningitis and pertussis. In an effort to track meningitis immunizations, all students need to fill out a meningitis tracking form. Meningitis is a potentially fatal or devastating illness and the risk is higher for college students living in residence halls than for 18–23 year old persons not in college. The vaccination is available at ASU health services.

Are you under the age of 18?

Arizona law requires parental consent to provide medical treatment for individuals under the age of 18. If you are under the age of 18, a Parental Consent form must be completed by your parent or guardian.
MMR documentation must be submitted through postal mail, email, fax or in person to ASU Health Services.
